Re cause of death: in US it is determined by the doctor. There is no “one way” of doing it. However, in most of the cases for covid related deaths there will be something else (e.g. pneumonia) plus covid19. Of course one can also code heart failure and covid if the test shows covid. So to get more accurate numbers, you need to look at known clinical patterns (e.g, pneumonia plus covid); and to get scary numbers — just get all covid cases.
